Pet Travel Form

Please complete this form in advance of your appointment. Please make sure you tick the declaration at the end of this form.

We will need you to bring proof of rabies vaccination and proof of the pet identification to your appointment. If you have a pet passport that was issued before 31.12.2020 this will suffice. If not, then please bring your vaccination card that shows the current rabies vaccination, and a document confirming that your pet has been microchipped before your appointment.

It is Crucial that the registered owner of the pet (i.e. person named on our system) is the person specifically named on the forms and will be travelling with the pet, is the person attending the AHC appointment. If someone else turns up, we cannot proceed and there will be a cancellation fee charged.
